Jotform submissions, called by the right person
Add the URL LumaKol gives you to your form’s WebHooks integration. Each submission becomes a lead, with Jotform’s name, phone and address fields filled in properly, and goes to a caller.

Who it’s for
Clinics, schools, event organisers, NGOs and service businesses that use Jotform for registrations, appointment requests and applications, often on a tablet at the front desk. Each submission that needs a callback becomes a lead in a caller’s queue.
- Jotform’s Full Name field arrives as one name, and its Phone field as one number with the area code.
- An Address field fills the lead’s street address, city, state and postal code.
- Dates and every other answer are saved as a note.
How to connect it

Add Jotform as a lead source
In LumaKol, go to Setup → Lead sources, choose Add source, then Jotform, and copy the webhook URL.

Add the WebHooks integration
Open the form in the Jotform Form Builder, go to Settings → Integrations, search for WebHooks, paste the URL and click Complete Integration.

Check the form fields
Make sure the form has a Phone field. Fields labelled like name, email, company or city fill those lead fields.

Choose who gets the leads
Give submissions to one person or share them in turn, set the starting stage and tags, then submit a test entry; it appears under Activity.
What comes in with each submission
- Name and phone from Jotform’s Full Name and Phone fields, and the email
- Street address, city, state and postal code from an Address field
- The form’s title, saved as the lead’s campaign
- Every other answer, dates included, as a note
After the lead arrives
Jotform’s own fields, such as form IDs and the visitor’s IP address, are left out. A submission is added once even if it is sent again, and a person who is already a lead gets the new submission as a note on their lead instead of a duplicate.
Questions
How quickly does a Jotform submission arrive?+
As soon as Jotform sends it, normally within seconds of the submission. The caller gets a push notification as the lead is added.
Does the form need a phone field?+
Yes. A submission without a phone number is not added, because every lead needs a number to call. The activity log shows it as Not added.
Will Jotform still email me each submission?+
Yes. The WebHooks integration is separate from your form’s email notifications, which carry on as before.
Can I connect several forms?+
Yes. Add the WebHooks integration with the same URL to each form; the form’s title is saved as each lead’s campaign.
