Jotform submissions, called by the right person
Add the URL LumaKol gives you to your form’s WebHooks integration. Each submission becomes a lead, with Jotform’s name, phone and address fields filled in properly, and goes to a caller.
- Name, phone and address fields understood
- Form title saved as campaign
- Every other answer kept

Jotform at a glance
- How leads get in
- Jotform’s WebHooks integration posts each submission to the private URL your LumaKol source gives you.
- How fast
- As Jotform sends the webhook, normally seconds after the form is submitted.
- Repeats and retries
- Each submission is recorded by its Jotform submission ID, so a resent submission is added once. A number that is already a lead gets a note.
- Where you set it up
- In the Jotform Form Builder: Settings → Integrations → WebHooks, paste the URL, then Complete Integration.
Who it’s for
Clinics, schools, event organisers, NGOs and service businesses that use Jotform for registrations, appointment requests and applications, often on a tablet at the front desk. Each submission that needs a callback becomes a lead in a caller’s queue.
- Jotform’s Full Name field arrives as one name, and its Phone field as one number with the area code.
- An Address field fills the lead’s street address, city, state and postal code.
- Dates and every other answer are saved as a note.
How to connect it
- 1
Add Jotform as a lead source
In LumaKol, go to Integrations → Lead sources, choose Add source, then Jotform, and copy the webhook URL.
- 2
Add the WebHooks integration
Open the form in the Jotform Form Builder, go to Settings → Integrations, search for WebHooks, paste the URL and click Complete Integration.
- 3
Check the form fields
Make sure the form has a Phone field. Fields labelled like name, email, company or city fill those lead fields.
- 4
Submit a test entry
Fill in the live form once with a real number. The submission shows under the source’s Activity as New lead, with the address split into its fields.
What comes in with each submission
| What Jotform sends | Where it lands in LumaKol |
|---|---|
| A Full Name field, with prefix, first, middle and last name joined | Name |
| A Phone field, as the full number or the area code and number together | Phone |
| An Email field | |
| Address field: street address lines 1 and 2 | Address |
| Address field: city, state and postal code | City, state and postal code |
| Address field: country | A “Country:” line in the note |
| Date fields | Note lines written as day-month-year |
| The form’s title | Campaign |
Jotform puts the answers in a block keyed by question number and field name; LumaKol reads that block, so Jotform’s own details such as the form ID, the visitor’s IP address and your username stay out of the lead.
After the lead arrives
The submission lands with the address already split into its fields, so the caller can confirm a visit or a delivery address from the lead itself. The push names the source, so name it after the form, and give a form its own Jotform source when it needs a different caller.
Common Jotform problems, and the fix
The phone number is missing its area code
A Phone field set to separate area code and number is joined back into one number. A number entered without a country code is read with your workspace’s country, so ask people outside that country to include their country code.
A submission shows as Not added
It had no phone number LumaKol could call. Add a Phone field to the form and make it required; the activity log shows the reason next to every entry that was not added.
Address details landed in the note
Separate boxes are matched by each field’s name in Jotform, such as city or address, and a box with an unusual name is kept as a note line. Jotform’s Address field always fills the lead’s address, city, state and postal code.
Several forms use the same webhook
That works: each lead carries its form’s title as the campaign. Give a form its own Jotform source when it needs a different caller, stage or tags.
Questions about Jotform
Does the Address field fill the lead’s address?+
Yes. Street lines go to the address, and city, state and postal code to their own fields. The country is kept as a line in the note.
Will Jotform still email me each submission?+
Yes. The WebHooks integration is separate from your form’s email notifications, which carry on as before.
Can I connect several forms?+
Yes. Add the WebHooks integration with the same URL to each form; the form’s title is saved as each lead’s campaign.
